This form is filled out at registration. Name and birthdate must match the birth certificate: don’t use nicknames. The club keeps the form. Most coaches make a copy. The registration page contains information about expenses, mailing addresses, and registration dates.

Loan forms are required when players, registered to one team, play for another team in a tournament. Loans are not permitted for league play. Loan forms must be signed by parent and both coaches. Loan forms do not require club, league or state signatures unless the tournament is held out-of-district or out-of-state. (For SC del Sol teams, this means that the additional signatures are required for tournaments in Tucson, Lake Havasu, Sierra Vista, California, and New Mexico.)

This form is used to document and process drops and transfers. Complete the following fields: effective date; player information (name, address, birthdate, phone number); current team; new team. Get signatures from current coach (and new coach, for transfers). Get signatures from current club registrar (and new club registrar, if transfer). Get parent's or guardian's signature. Attach the current player pass; for transfers, the new coach can clip the photo to use with the new pass. Coaches and parents can't take the form to VYSL or the state association directly. Take the form to the club registrar for processing.
Completed applications, including travel rosters, must be submitted to the state association at least one week before the out-of-state event.
Completed forms must be submitted to the club registrar within seven days from the injury. The registrar will submit the form to the league and state administrators. Timely completion protects your insurance status, should you eventually decide to submit a claim.
Players born outside the United States, whose proof of age is a passport or foreign birth certificate, and who can't show that they resided in the U.S. before their fourteenth birthday, must get clearance from their country of origin’s soccer association. Before the season begins, submit the form to the Arizona Youth Soccer Association. You should plan for processing -- including the check at the country of origin’s national association -- to take at least thirty days. There is no fee for this process.
